What is insulin resistance and how does it develop?

Insulin resistance occurs when cells — particularly in muscle, fat, and liver — respond less effectively to insulin, requiring the pancreas to produce more to maintain normal blood sugar. It develops through a combination of excess adiposity, physical inactivity, genetics, and chronic inflammation.

What is the difference between insulin resistance and diabetes?

Insulin resistance is a precursor state — blood sugar may still be normal or mildly elevated (prediabetes) because the pancreas is compensating. Type 2 diabetes develops when the pancreas can no longer maintain sufficient insulin output to overcome resistance.

Can insulin resistance be reversed?

Yes — particularly in earlier stages. Weight loss, regular physical activity, dietary changes prioritizing lower glycemic foods, and improved sleep quality can substantially improve insulin sensitivity. Some people achieve normalization of blood sugar without medication.

Do I need medication for insulin resistance?

Not always immediately. Lifestyle intervention is the foundation of treatment. Metformin is commonly prescribed when lifestyle changes are insufficient or when prediabetes is present with additional risk factors. Newer agents may also be considered depending on the clinical picture.

What side effects are associated with insulin resistance treatments?

Metformin is well tolerated — the most common side effects are GI symptoms, especially when started. Taking it with food minimizes this. Rare but important: metformin should be paused around contrast dye procedures and is not recommended with significantly impaired kidney function.
