What is Indocin (indomethacin)?

Indocin is a potent non-steroidal anti-inflammatory drug (NSAID) used to treat moderate to severe pain and inflammation in conditions including rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, acute gout flares, and bursitis. It's one of the most powerful NSAIDs available and is particularly valued for acute gout because of how rapidly and effectively it reduces the intense joint inflammation of a gout attack. It works by inhibiting COX-1 and COX-2 enzymes to reduce prostaglandin production. Because of its potency, it carries a higher risk of GI side effects than milder NSAIDs and is typically used at the lowest effective dose for the shortest duration.

Potent COX-1 and COX-2 inhibition

Blocks both cyclooxygenase isoforms with high potency, dramatically reducing prostaglandin synthesis and delivering powerful anti-inflammatory, analgesic, and antipyretic effects stronger than most other oral NSAIDs.

Acute gout flare management

Highly effective for the intense inflammatory arthritis of acute gout—Indomethacin's rapid onset and potent anti-inflammatory activity can significantly reduce the severe joint pain and swelling within hours of dosing.

Higher GI risk consideration

Its high COX-1 inhibitory activity reduces the protective prostaglandins in the gastric lining, making GI irritation, ulceration, and bleeding more likely than with less potent NSAIDs—taken with food and used for the shortest effective duration.

Frequently Asked Questions

What conditions is Indocin used for?

Indocin is a potent NSAID prescribed for acute gout attacks, osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, bursitis, and tendinitis — conditions where its high anti-inflammatory potency provides an advantage over milder NSAIDs.

How quickly does Indocin work for a gout attack?

For acute gout, meaningful pain relief typically begins within hours of the first dose. The rapid anti-inflammatory action makes it one of the most effective agents for stopping an acute gout flare quickly.

Should I take Indocin with food?

Yes — always take indomethacin with food, milk, or antacids. It has one of the highest rates of GI side effects among NSAIDs, and taking it with food significantly reduces the risk of stomach upset, ulceration, and bleeding.

What side effects might I experience?

Stomach pain, nausea, heartburn, dizziness, and headache are most common. Indocin has a higher rate of GI side effects than most other NSAIDs due to its non-selective COX inhibition — use at the lowest effective dose for the shortest time.

Can I take Indocin if I have kidney disease?

NSAIDs reduce kidney blood flow and should be used with caution or avoided in people with existing kidney disease. Indomethacin's high potency makes this consideration particularly important — discuss with your clinician.
