What is Hyzaar (Losartan - hydrochlorothiazide)?

Hyzaar is a combination blood pressure medication containing losartan, an angiotensin II receptor blocker (ARB), and hydrochlorothiazide (HCTZ), a thiazide diuretic. Together, they lower blood pressure through two complementary mechanisms—losartan blocks vasoconstriction from angiotensin II while HCTZ reduces blood volume through increased kidney excretion of salt and water. The combination also helps counteract some of each drug's side effects: HCTZ tends to raise potassium loss, while losartan tends to retain it. It's taken once daily and is appropriate for patients who need dual therapy to achieve blood pressure control.

Dual angiotensin blockade and diuresis

Losartan blocks AT1 receptors to prevent angiotensin II-driven vasoconstriction, while hydrochlorothiazide reduces circulating blood volume by inhibiting sodium reabsorption in the kidney—attacking high blood pressure through two independent and additive mechanisms.

Complementary electrolyte balance

HCTZ causes potassium loss, while Losartan's aldosterone-blocking effect tends to conserve potassium—the two effects partially offset each other, producing a more balanced electrolyte profile than HCTZ used alone.

Stroke risk reduction in hypertension with LVH

Hyzaar has specific clinical trial evidence for reducing stroke risk in hypertensive patients with left ventricular hypertrophy—a cardiovascular benefit that extends beyond blood pressure lowering alone.

Frequently Asked Questions

Why were losartan and HCTZ combined in one pill?

Combining an ARB and a diuretic addresses blood pressure through two complementary mechanisms simultaneously — greater reduction than either alone — while simplifying the regimen to one daily pill.

What specific cardiovascular benefit does Hyzaar have beyond blood pressure?

Clinical trial data specifically shows Hyzaar reduces the risk of stroke in hypertensive patients with left ventricular hypertrophy — a more targeted outcome benefit than most blood pressure combination pills can claim.

Does the losartan component protect my kidneys?

Yes. Losartan specifically reduces progression of diabetic nephropathy in patients with Type 2 diabetes and proteinuria — a clinically important benefit for the large number of hypertensive patients who also have diabetes.

What side effects might I experience?

Dizziness upon standing, elevated potassium from losartan, low potassium from HCTZ (the two effects partly offset each other), elevated uric acid that may trigger gout, and increased urination are the most commonly reported effects.

Can I take Hyzaar during pregnancy?

No. Losartan is contraindicated in pregnancy and can cause serious fetal kidney and developmental harm. Stop taking it immediately and contact your clinician if pregnancy occurs while on Hyzaar.
