Who is a good candidate for hormone replacement therapy?

HRT is most appropriate for healthy women under 60 who are within 10 years of menopause onset and have significant vasomotor symptoms (hot flashes, night sweats), vaginal symptoms, or impaired quality of life. Individual cardiovascular health, breast cancer history, and thrombosis risk inform candidacy.

What types of HRT are available?

Systemic HRT — delivered as pills, patches, gels, or sprays — treats vasomotor and systemic menopausal symptoms. Local vaginal estrogen treats genitourinary symptoms only, with minimal systemic absorption. Women with a uterus require combined estrogen-progestogen therapy; those without a uterus use estrogen alone.

Is HRT safe after breast cancer?

Systemic HRT is generally contraindicated in women with hormone receptor-positive breast cancer. Individual cases — particularly hormone receptor-negative cancers or severe quality-of-life impact — require careful specialist discussion. Local vaginal estrogen at low doses may be considered in some breast cancer survivors with clinical guidance.

How long can I take HRT?

Current guidance has shifted away from arbitrary time limits — the decision is individualized. For women without specific risk factors, continuing HRT while it provides benefit and the benefit-risk balance remains favorable is reasonable. Annual reassessment with your clinician is recommended.

What side effects are associated with HRT?

Estrogen may cause breast tenderness, bloating, and nausea. Combined HRT with synthetic progestins carries a small increased risk of breast cancer with long-term use — micronized progesterone may have a more favorable profile. Estrogen-only HRT slightly increases uterine cancer risk in women with a uterus — which is why progestogen is added.
