What is Hiprex (methenamine)?

Hiprex is a prescription urinary antiseptic used for the long-term prevention of recurring urinary tract infections (UTIs) in patients who experience frequent infections. It works differently from antibiotics—rather than killing bacteria directly, it releases formaldehyde in acidic urine, which creates a hostile environment that suppresses bacterial growth in the bladder. Because it works locally in the urinary tract and doesn't enter systemic circulation significantly, it avoids the antibiotic resistance concerns associated with long-term antibiotic use. It's taken twice daily and works best when urine is kept acidic.

Formaldehyde conversion

Methenamine is converted to formaldehyde in acidic urine, creating a bacteriostatic environment within the bladder that inhibits bacterial growth and adherence without systemic antibiotic exposure.

Resistance-sparing prevention

Because it doesn't work as a traditional antibiotic, long-term Hiprex use doesn't contribute to antibiotic resistance—a major advantage over continuous low-dose antibiotic prophylaxis for recurrent UTIs.

Urine acidification dependency

Works best when urinary pH is low. Vitamin C or other acidifying agents are sometimes used alongside Hiprex to maintain the acidic urine environment needed for optimal formaldehyde release.

Frequently Asked Questions

How is Hiprex different from antibiotic UTI prevention?

Hiprex prevents UTIs through a non-antibiotic mechanism — it releases formaldehyde in acidic urine, creating a hostile environment for bacteria. It does not contribute to antibiotic resistance, unlike long-term antibiotic prophylaxis.

How acidic does my urine need to be for it to work?

Hiprex requires urine pH below 6 to generate effective formaldehyde levels. Your clinician may recommend vitamin C or other acidifying agents alongside Hiprex to help maintain the required urine acidity.

How long before it starts preventing UTIs?

Hiprex works as an ongoing preventive taken twice daily — it maintains a continuously bacteriostatic urine environment rather than treating active infections. Regular use over weeks to months is needed to assess its effectiveness.

What side effects might I experience?

Nausea, stomach upset, and bladder irritation are the most commonly reported side effects. High doses can cause more pronounced GI discomfort. These are generally mild and manageable at standard therapeutic doses.

Can Hiprex be used during pregnancy?

Hiprex is generally avoided in the first trimester and used cautiously thereafter. If you are pregnant or planning pregnancy, discuss the risks and benefits carefully with your clinician before continuing use.
