Frequently Asked Questions

What is hidradenitis suppurativa and what causes it?

Hidradenitis suppurativa (HS) is a chronic inflammatory skin condition causing painful nodules, abscesses, and tunneling tracts — most commonly in the armpits, groin, buttocks, and under the breasts. It involves inflammation of hair follicles rather than sweat glands, despite the name. Genetics, smoking, and obesity are significant risk factors.

Is HS a hygiene problem?

No — HS is not caused by poor hygiene and is not contagious. This is one of the most important misconceptions to address, as it contributes to significant stigma and delays patients from seeking care. HS is an inflammatory condition with genetic and immune components.

What treatments are available for hidradenitis suppurativa?

Treatment is staged by severity. Mild HS is managed with topical clindamycin, antibacterial washes, and lifestyle modifications. Moderate disease may require oral antibiotics (typically doxycycline) or hormonal therapy. Severe or refractory HS is treated with biologics — adalimumab (Humira) and secukinumab are FDA-approved for HS.

Does weight loss help with hidradenitis suppurativa?

Yes — weight loss reduces friction, sweating, and inflammatory burden in skin folds, and is associated with meaningful improvement in HS severity. Smoking cessation is also strongly recommended. These lifestyle changes complement medical treatment and may reduce medication requirements.

What side effects are associated with HS treatments?

Long-term oral antibiotics carry GI effects and resistance risks. Adalimumab and other biologics require monitoring for infections and immune effects — including reactivation of latent TB before starting. Surgical procedures for tunneling tracts cause wound care requirements and scarring. Deroofing procedures have good outcomes when performed by experienced surgeons.
