What is the difference between HSV-1 and HSV-2?

HSV-1 primarily causes oral herpes (cold sores) and is transmitted through oral contact — though it increasingly causes genital herpes through oral-genital contact. HSV-2 primarily causes genital herpes and is transmitted through sexual contact. Both types cause lifelong latent infection with periodic reactivations.

Can herpes be transmitted when there are no visible symptoms?

Yes — asymptomatic shedding is the primary mode of transmission. People with genital herpes shed the virus from genital skin on approximately 10–20% of days without any symptoms or visible lesions. This makes disclosure to partners and discussion of risk reduction strategies important.

Is there a cure for herpes?

There is no cure — HSV establishes lifelong latency in nerve ganglia and cannot be eliminated. Antiviral medications (acyclovir, valacyclovir, famciclovir) suppress replication, reduce outbreak frequency and severity, and significantly reduce — though do not eliminate — transmission risk to partners.

How does daily suppressive therapy differ from episodic treatment?

Episodic therapy treats individual outbreaks when they occur. Daily suppressive therapy reduces outbreak frequency by 70–80%, decreases viral shedding, and reduces transmission risk to uninfected partners by approximately 50%. Suppressive therapy is particularly beneficial for people with frequent outbreaks or serodiscordant relationships.

What side effects are associated with herpes treatments?

Valacyclovir and acyclovir are very well tolerated — headache and nausea are the most common effects. Rare kidney toxicity with high doses requires adequate hydration. Long-term suppressive therapy at standard doses is considered very safe based on decades of use data. Famciclovir has a similar favorable safety profile.
