Heavy menstrual bleeding (menorrhagia) is clinically defined as blood loss greater than 80 mL per cycle, though this is impractical to measure. Practical indicators include soaking through a pad or tampon every hour for several consecutive hours, passing clots larger than a quarter, or bleeding lasting more than 7 days.

What causes heavy periods?

Common causes include uterine fibroids, polyps, adenomyosis, hormonal imbalance (particularly anovulatory cycles), thyroid disorders, bleeding disorders (von Willebrand disease), and certain medications including anticoagulants and NSAIDs. Structural causes are identified with pelvic ultrasound.

Do heavy periods always require treatment?

Treatment is indicated when heavy bleeding causes anemia, significantly impacts quality of life, or has an identifiable correctable cause. For women not bothered by their flow and without anemia, monitoring is reasonable. The decision to treat and what treatment to use depends on cause, severity, and reproductive plans.

Can hormonal treatment control heavy periods without surgery?

Yes — the levonorgestrel IUD (Mirena) is the most effective non-surgical treatment for heavy menstrual bleeding, reducing flow by 80–90% in most users. Combined oral contraceptives, progestins, and tranexamic acid are other effective options. Treatment choice is guided by cause and individual factors.

What side effects are associated with heavy flow treatments?

The levonorgestrel IUD may cause irregular spotting in the first 3–6 months before flow reduction. Oral contraceptives may cause nausea, breast tenderness, and mood changes. Tranexamic acid may cause GI upset. Surgical options (endometrial ablation, myomectomy) carry procedural risks discussed at consultation.
