What is H. pylori and why does it matter?

Helicobacter pylori is a bacterial infection of the stomach lining present in roughly half the world's population. It is the leading cause of peptic ulcers and chronic gastritis, significantly increases gastric cancer risk, and is a treatable cause of functional dyspepsia. Testing and treating it when present is clinically important.

How is H. pylori tested?

Non-invasive options include the urea breath test (most accurate), stool antigen test, and blood antibody test (less useful as it can't distinguish active from past infection). Endoscopy with biopsy is used when upper GI evaluation is also needed. Breath and stool tests require stopping PPIs and antibiotics beforehand.

Who should be tested for H. pylori?

Testing is recommended for patients with active or past peptic ulcer disease, functional dyspepsia, before starting long-term NSAID use, with unexplained iron deficiency anemia, and in some guidelines for first-degree relatives of gastric cancer patients. A 'test and treat' strategy is widely recommended for uninvestigated dyspepsia.

What happens if I test positive?

A positive test is treated with a combination antibiotic regimen — typically a 10–14-day course including two antibiotics and a proton pump inhibitor. Test-of-cure is recommended 4 weeks after completing treatment to confirm eradication, as treatment failure and antibiotic resistance affect outcomes.

What side effects are associated with H. pylori treatment?

Triple or quadruple therapy regimens frequently cause GI upset, nausea, metallic taste, and diarrhea. Antibiotic-associated disruption of gut flora is common.
