How did I get H. pylori?

H. pylori transmission is not fully understood but primarily occurs through oral-oral or fecal-oral routes — contaminated water, food, or close contact in childhood. Most infections are acquired in childhood, often in settings with limited access to clean water. It's not typically spread through casual adult contact.

What problems can H. pylori cause if left untreated?

Untreated H. pylori causes chronic gastritis, peptic ulcer disease (in 10–15% of infected individuals), and significantly increases the risk of gastric cancer — the third leading cause of cancer death worldwide. Eradicating it reduces ulcer recurrence and lowers gastric cancer risk.

How is H. pylori treated?

Standard treatment is a 10–14-day combination regimen — typically a proton pump inhibitor with two antibiotics (clarithromycin-based triple therapy or bismuth quadruple therapy). Treatment success is confirmed with a breath or stool test at least 4 weeks after completing the regimen.

What if my first treatment course doesn't work?

Treatment failure occurs in 15–20% of cases — often due to antibiotic resistance, particularly to clarithromycin. Second-line regimens use different antibiotic combinations. Antibiotic susceptibility testing via culture or molecular testing can guide salvage therapy in recurrent treatment failures.

What side effects are associated with H. pylori treatment?

Triple and quadruple therapy regimens frequently cause nausea, diarrhea, metallic taste, and abdominal discomfort. Antibiotic-associated gut flora disruption is common — probiotics taken concurrently may reduce GI side effects. Bismuth causes harmless black discoloration of stool.
