What is Forteo (Teriparatide)?

Forteo is a prescription injectable medication used to treat osteoporosis in postmenopausal women, men, and patients taking glucocorticoids who are at high risk for fracture. Unlike most osteoporosis medications that work by slowing bone loss, Forteo actually stimulates new bone formation—making it one of the few truly anabolic bone treatments available. It's a synthetic form of parathyroid hormone (PTH) that, when given as a once-daily injection, activates bone-building cells. Treatment is typically limited to 2 years, after which another bone-preserving medication is used to maintain the gains.

Anabolic bone formation stimulation

Activates PTH type 1 receptors on osteoblasts—the cells responsible for building new bone—stimulating them to produce and mineralize new bone matrix, increasing bone density and structural strength.

Fracture risk reduction

By building new bone rather than simply slowing its loss, Forteo produces meaningful increases in bone mineral density at the spine and hip, translating to significant reductions in vertebral and non-vertebral fracture risk.

Sequential therapy design

Used for up to 2 years to generate new bone mass, then followed by an antiresorptive medication like a bisphosphonate to preserve the gains—a strategic sequence that maximizes long-term skeletal benefit.

Frequently Asked Questions

How is Forteo different from other osteoporosis medications?

Most osteoporosis medications slow bone loss — Forteo actually stimulates new bone formation. This makes it one of the few truly anabolic bone-building treatments, specifically for people at high fracture risk.

How long can I take Forteo?

Forteo is approved for a maximum of 2 years of use. After completing the course, another bone-preserving medication — typically a bisphosphonate — is prescribed to maintain the gains in bone density.

What happens after I finish my Forteo course?

The bone density gains from Forteo can be lost relatively quickly without follow-up treatment. Starting an antiresorptive medication like alendronate or denosumab immediately after is strongly recommended.

What side effects might I experience?

Nausea, dizziness, and leg cramps are the most commonly reported effects, often occurring shortly after injection. Transient elevated blood calcium can occur. Injecting while sitting or lying down helps reduce dizziness.

How is Forteo administered?

Forteo is injected once daily using a prefilled pen device — into the thigh or abdomen. Rotating injection sites reduces local irritation. The pen is stored in the refrigerator and should not be frozen.
