What is Exjade (Deferasirox)?

Exjade is an oral iron chelator used to treat chronic iron overload caused by repeated blood transfusions in patients with conditions like thalassemia, sickle cell disease, myelodysplastic syndromes, and other anemias requiring regular transfusion therapy. Each unit of transfused blood adds iron that the body cannot excrete, and over time toxic iron accumulates in the liver, heart, and other organs. Deferasirox binds to iron in the bloodstream and tissues and carries it out of the body through urine and feces. It's dissolved in water or juice and taken once daily on an empty stomach.

Iron chelation and excretion

Deferasirox binds to free iron with high affinity and selectivity, forming stable complexes that are excreted primarily in the feces via bile and to a lesser degree in the urine—progressively reducing total body iron burden with daily use.

Organ protection from iron toxicity

Chronic iron overload damages the liver, heart, and endocrine organs through oxidative stress and direct cellular toxicity. By reducing iron stores, Exjade protects these organs from the progressive damage that untreated transfusional iron overload causes over time.

Once-daily oral convenience

Unlike older injectable chelators that required subcutaneous infusions over 8–12 hours, Deferasirox is taken as a once-daily dispersible tablet dissolved in liquid—dramatically improving patient convenience and adherence compared to infusion-based chelation therapy.

Frequently Asked Questions

What is Exjade used for?

Exjade removes excess iron that accumulates in the body from repeated blood transfusions — a condition called transfusional iron overload. Over time, excess iron damages the liver, heart, and endocrine organs if not removed.

How do I take Exjade correctly?

Dissolve the tablet completely in water, apple juice, or orange juice and drink immediately. Take it once daily on an empty stomach, 30 minutes before eating. Do not chew or swallow the tablet whole — it must be fully dissolved.

How quickly does Exjade reduce iron levels?

Iron levels — measured by serum ferritin and liver iron concentration — typically begin declining within the first few months of consistent daily use. Regular monitoring guides dose adjustments to maintain effective iron reduction.

What side effects might I experience?

Nausea, stomach pain, diarrhea, skin rash, and headache are most common. Kidney function and liver enzymes require regular monitoring — Exjade can cause kidney and liver toxicity that may require dose adjustment or discontinuation.

Does Exjade require regular blood tests?

Yes — monthly serum creatinine, liver enzymes, and periodic serum ferritin and complete blood counts are recommended. Kidney function monitoring is especially important as Exjade can cause dose-dependent kidney effects.
