What does an estrogen test measure, and which type of estrogen is tested?

Most clinical estrogen tests measure estradiol (E2) — the most potent and clinically relevant form of estrogen in reproductive-age and perimenopausal women. Estradiol reflects ovarian function, guides fertility assessment, and helps evaluate menopausal status and HRT monitoring.

When during my cycle should I have estrogen tested?

Timing depends on the purpose. Day 2–3 of the menstrual cycle is standard for fertility assessment (baseline estradiol). For monitoring estrogen therapy or assessing perimenopausal status, timing is less critical and can be discussed with your clinician based on the clinical question.

Do I need to fast for an estrogen test?

No fasting is required. Estradiol levels are not meaningfully affected by recent food intake.

Can estrogen testing help explain my symptoms?

Yes — low estradiol can explain hot flashes, night sweats, vaginal dryness, irregular periods, and mood changes. High estradiol may be associated with heavy periods, fibroid growth, or endometriosis. Results need to be interpreted in the context of your symptoms and menstrual cycle phase.

I'm on HRT or hormonal contraception — will the test still be useful?

Yes — estradiol testing while on therapy shows whether your current dose is achieving therapeutic levels. For women on HRT, a result in the appropriate range confirms the regimen is working as intended. Inform your clinician of any hormonal medications when reviewing results.
