What is an epidermoid cyst and is it dangerous?

An epidermoid cyst is a benign, slow-growing sac filled with keratin — forming just beneath the skin surface. Despite the common name 'sebaceous cyst,' most are actually epidermoid in origin. They are not dangerous or precancerous, but can become inflamed, infected, or cosmetically bothersome.

Will an epidermoid cyst go away on its own?

Most epidermoid cysts do not resolve spontaneously — they typically persist and may gradually enlarge over time. They only require treatment when inflamed, infected, or causing functional or cosmetic concern. Watchful waiting is appropriate for stable, asymptomatic cysts.

What happens when an epidermoid cyst gets infected?

An infected cyst becomes red, warm, tender, and swollen — and may spontaneously drain. Acute infected cysts are typically treated with incision and drainage plus antibiotics if cellulitis is present. Definitive excision is performed electively after the infection resolves — excising an acutely infected cyst significantly increases recurrence risk.

Is squeezing or popping an epidermoid cyst safe?

Squeezing a cyst ruptures it into the surrounding tissue — triggering an intense inflammatory reaction that causes more pain and swelling than the original cyst, and significantly increases infection risk. It does not remove the cyst wall, so the cyst will recur. Leaving cysts alone until formal excision is strongly preferred.

What side effects are associated with epidermoid cyst treatments?

Incision and drainage relieves acute symptoms but has high recurrence rates — the cyst wall must be removed for definitive treatment.
