What is endometriosis and what causes it?

Endometriosis is a condition where tissue similar to the uterine lining grows outside the uterus — on the ovaries, fallopian tubes, bladder, bowel, or peritoneum. It causes chronic pelvic pain, painful periods, pain with intercourse, and fertility problems. The exact cause remains incompletely understood.

How is endometriosis diagnosed?

Definitive diagnosis requires laparoscopic surgery with biopsy — clinical diagnosis based on symptoms and imaging has significant limitations. Pelvic MRI and ultrasound can identify endometriomas (ovarian cysts) and deep infiltrating endometriosis but cannot rule out the condition. Diagnostic delay of 7–10 years is common due to symptom normalization.

Can endometriosis affect my fertility?

Yes — endometriosis is associated with infertility through multiple mechanisms including distorted pelvic anatomy, ovarian reserve reduction from endometriomas, and inflammatory effects on embryo implantation. Fertility impact varies significantly by stage and location. Early evaluation and treatment optimize reproductive outcomes.

What are the treatment options for endometriosis?

Management ranges from NSAIDs and hormonal suppression (combined oral contraceptives, progestins, GnRH agonists/antagonists) to laparoscopic surgical excision of lesions. Hormonal treatment suppresses symptoms but does not eliminate disease. Surgical excision is preferred for improving fertility and providing definitive symptom relief.

What side effects are associated with endometriosis treatments?

Hormonal contraceptives may cause mood changes, spotting, and decreased libido. GnRH agonists cause menopausal symptoms and bone density loss — add-back hormonal therapy mitigates this. Elagolix (Orilissa) may cause bone density reduction and mood changes. Surgical treatment carries procedural risks and possible recurrence.
