What is Emgality (Galcanezumab)?

Emgality is a CGRP monoclonal antibody injection used for the preventive treatment of migraine in adults and for the treatment of episodic cluster headache. It works by binding to and blocking calcitonin gene-related peptide (CGRP), a neuropeptide that is central to the initiation of migraine and cluster headache attacks. By neutralizing CGRP before it can trigger pain signaling, Emgality reduces both the frequency and severity of attacks over time. For migraine prevention, it's injected once monthly; for cluster headache, a higher loading dose is given at the start of a cluster period.

CGRP ligand neutralization

Binds directly to the CGRP molecule—rather than its receptor—preventing it from triggering the trigeminal inflammatory cascade that initiates migraine and cluster headache pain.

Migraine and cluster headache coverage

One of the few treatments approved for both episodic migraine prevention and episodic cluster headache—a severe, poorly-treated headache disorder where treatment options have historically been very limited.

Once-monthly preventive dosing

A single monthly subcutaneous injection provides continuous CGRP neutralization and sustained reduction in attack frequency—significantly simpler than daily oral preventive medications and with effects maintained as long as treatment continues.

Frequently Asked Questions

How is Emgality different from triptans?

Emgality prevents migraines from occurring — it's not used to stop an attack in progress. Triptans are taken acutely during an attack. They serve completely different roles and can be used together in a comprehensive migraine plan.

How quickly does Emgality reduce migraine frequency?

Many people notice a reduction in migraine days within the first month of treatment. Clinical trials showed continued improvement over 3–6 months of consistent monthly dosing.

Is Emgality also approved for cluster headaches?

Yes. Emgality is FDA-approved for both episodic migraine prevention and episodic cluster headache — one of the few preventive treatments approved for cluster headache, which is notoriously difficult to treat.

What side effects might I experience?

Injection site reactions — pain, redness, and bruising — are the most commonly reported effects. Constipation has been reported in a subset of patients. Serious systemic side effects are uncommon given the targeted mechanism.

How long do I need to stay on Emgality to benefit?

Preventive benefit builds over the first 3–6 months. Most clinicians reassess response after 3–6 months to determine whether continued treatment is appropriate and whether the frequency of migraines has meaningfully decreased.
