What is Elimite (permethrin cream)?

Elimite is a prescription-strength 5% permethrin cream used to treat scabies—a highly contagious skin infestation caused by tiny mites that burrow into the skin and cause intense itching. It's applied from the neck down to the entire body, left on for 8–14 hours, and then washed off—typically as a single-treatment application. Permethrin is a synthetic pyrethroid that kills both adult scabies mites and their eggs by disrupting their nervous system. Prescription-strength Elimite is more concentrated than OTC permethrin products and is the first-line treatment for scabies.

Sodium channel disruption in mites

Permethrin prolongs the opening of sodium channels in the nerve cell membranes of scabies mites, causing continuous nerve firing, paralysis, and death of both adult mites and their eggs.

Ovicidal action

Kills not only adult Sarcoptes scabiei mites but also their eggs—reducing the likelihood of reinfestation from eggs that survive a treatment that only affects adult mites.

Is one application of Elimite enough?

For most people, a single treatment application left on overnight is sufficient to clear scabies. A second treatment 7–14 days later may be recommended if live mites are still observed after the first course.

Does the whole body need to be treated?

Yes. Elimite is applied from the neck down to the entire body surface, including between fingers and toes, under nails, and in skin folds. Thorough, complete coverage is essential for effective treatment.

Should my household contacts also be treated?

Yes. Because scabies spreads through close personal contact, all household members and close contacts should be treated simultaneously — even those without symptoms — to prevent reinfestation.

What side effects might I experience?

Temporary burning, stinging, itching, and redness at the application site are common. Itching may continue for 2–4 weeks after successful treatment as the immune system responds to dead mites — this doesn't mean the treatment failed.

How do I prevent reinfestation after treatment?

Wash all clothing, bedding, and towels used in the 3 days before treatment in hot water. Items that can't be washed can be sealed in plastic bags for 72 hours, as mites can't survive off the body that long.
