What causes earwax impaction?

Cerumen (earwax) impaction occurs when wax accumulates faster than it naturally migrates out of the ear canal. Contributing factors include using cotton swabs (which push wax deeper), narrow ear canals, hearing aid use, overproduction of cerumen, and aging-related changes in wax consistency.

Does earwax need to be removed?

Most earwax migrates out of the ear naturally and doesn't need removal. Earwax only requires treatment when it causes symptoms — hearing reduction, fullness, tinnitus, ear pain, or dizziness — or when it prevents adequate ear examination. Routine removal of asymptomatic earwax is not recommended.

Can I remove earwax at home safely?

Ear drops — mineral oil, hydrogen peroxide, or commercial cerumenolytics — can soften wax for self-care of mild accumulation. Cotton swabs should never be inserted into the ear canal — they push wax deeper and risk canal injury. Ear candling is not safe or effective and is not recommended.

When should earwax removal be performed by a clinician?

Clinical removal is appropriate when home softening drops haven't resolved symptoms, when impaction is causing significant hearing loss or discomfort, or when the ear needs to be examined for another reason. Irrigation, manual removal with a curette, or suction are the standard clinical methods.

What side effects are associated with earwax removal?

Ear drops may cause mild temporary stinging — this is normal. Clinical irrigation carries a very small risk of tympanic membrane perforation in patients with ear disease. Manual removal with instruments is very safe when performed by a trained clinician. Dizziness from water temperature during irrigation is the most common transient side effect.
