How do I know if I have a middle ear infection or an outer ear infection?

Middle ear infection (otitis media) causes pain behind the eardrum, often with fever, muffled hearing, and sometimes fluid drainage. Outer ear infection (otitis externa, 'swimmer's ear') causes pain with movement of the ear canal or tragus, itching, and discharge. Treatment differs — one requires oral antibiotics, the other topical.

Do ear infections always need antibiotics?

Not always. Many uncomplicated middle ear infections in adults resolve on their own within 2–3 days. Current guidelines support a watchful waiting approach in appropriate patients with mild symptoms. Antibiotic treatment is recommended when symptoms are severe, bilateral, or don't improve within 48–72 hours.

What can I do to manage ear pain while waiting for treatment to work?

OTC pain relievers — ibuprofen or acetaminophen — are effective for ear pain management. Warm compresses over the ear may provide additional comfort. Avoid putting anything in the ear canal unless directed by your clinician.

When should I seek same-day evaluation for an ear infection?

Promptly seek evaluation for high fever with ear pain, sudden hearing loss, dizziness or balance problems, facial weakness, pain spreading beyond the ear, or significant swelling behind the ear — these may indicate complications requiring urgent assessment.

What side effects are associated with ear infection treatments?

Oral antibiotics (amoxicillin is most commonly used) may cause diarrhea, nausea, and rash. Topical antibiotic ear drops are very well tolerated with minimal systemic absorption.
