What is Doxepin?

Doxepin is a tricyclic antidepressant that, at higher doses, treats depression and anxiety, and at very low doses is FDA-approved specifically for insomnia characterized by difficulty staying asleep. Its sleep-promoting effects come from its potent antihistamine activity, which produces sedation without the dependency risks of traditional sleep medications. At the low doses used for insomnia (3–6 mg), it's one of the few medications approved for sleep maintenance insomnia—the frustrating pattern of waking up in the middle of the night or too early in the morning.

Histamine H1 receptor blockade

At low doses, Doxepin's powerful antihistamine action produces robust sedation by blocking H1 receptors in the brain, extending sleep duration without the tolerance, dependence, or next-day impairment of many other sleep medications.

Sleep maintenance focus

Uniquely approved for sleep maintenance insomnia (difficulty staying asleep), rather than just sleep onset—addressing the middle-of-night and early morning awakening patterns that many patients struggle with most.

Non-controlled sleep option

Not a controlled substance at any dose, meaning it can be prescribed more freely and used longer-term without the regulatory restrictions and dependency concerns that apply to benzodiazepines and Z-drugs.

Frequently Asked Questions

How is doxepin for sleep different from doxepin for depression?

It's a dose difference. At 3–6 mg, doxepin's antihistamine sedation is the primary effect and it's FDA-approved for insomnia. At 75–300 mg, it functions as a full tricyclic antidepressant — these are entirely different clinical uses.

How quickly does low-dose doxepin improve sleep?

Many people experience improved sleep onset and duration within the first few nights. Unlike antidepressant effects that take weeks to develop, the sleep-promoting properties of doxepin work relatively quickly from the start.

Is doxepin for sleep habit-forming?

Doxepin is not a controlled substance and is not considered habit-forming — an important advantage over benzodiazepines and Z-drugs. It can be used for longer periods without the dependency concerns of traditional sleep medications.

What side effects might I experience?

At low doses used for sleep, drowsiness, dry mouth, and dizziness are most common — particularly the next morning. Older adults are more sensitive to these effects and should use caution.

Can doxepin be used for sleep long-term?

Yes. Because it's non-addictive and not a controlled substance, doxepin can be used longer-term for chronic insomnia — a meaningful advantage over most prescription sleep aids that carry dependency concerns.
