What is diverticulosis and does it need treatment?

Diverticulosis is the presence of small outpouchings (diverticula) in the colon wall — present in more than half of adults over 60. Most people with diverticulosis never develop symptoms or complications. It does not require specific treatment but warrants dietary and lifestyle measures to reduce the risk of progression to diverticulitis.

Will I definitely get diverticulitis if I have diverticulosis?

No — approximately 75–80% of people with diverticulosis never develop diverticulitis. The annual risk of diverticulitis in someone with diverticulosis is approximately 1–2%. A high-fiber diet, adequate hydration, regular physical activity, and maintaining healthy weight are the most evidence-supported preventive measures.

Can diverticulosis cause pain even without diverticulitis?

Some patients with diverticulosis experience chronic lower abdominal discomfort or altered bowel habits without acute inflammation — sometimes called 'symptomatic uncomplicated diverticular disease.' This is distinct from IBS, though the two can overlap and share some management strategies.

Should I avoid certain foods with diverticulosis?

The traditional restriction of seeds, nuts, and popcorn is no longer supported by evidence — these foods do not increase diverticulitis risk and need not be avoided. A high-fiber diet (25–38g daily) is the most important dietary recommendation. Adequate hydration (at least 8 cups daily) supports fiber's beneficial effects.

What side effects are associated with diverticulosis management?

High-fiber diets and fiber supplements may cause initial bloating and gas — starting gradually and increasing intake over 2–4 weeks minimizes this. Adequate hydration reduces constipation associated with increased fiber. Fiber supplements are very safe. Mesalamine, used in some symptomatic cases, may cause GI side effects.
