Frequently Asked Questions

What is diverticulitis and how is it different from diverticulosis?

Diverticulosis is the presence of small pouches (diverticula) in the colon wall — common with aging and usually asymptomatic. Diverticulitis occurs when one or more of these pouches becomes inflamed or infected — causing left lower abdominal pain, fever, and changes in bowel habits that require clinical treatment.

Does diverticulitis always need antibiotics?

Recent evidence challenges the routine use of antibiotics for uncomplicated acute diverticulitis in otherwise healthy patients — some guidelines now support a watchful waiting approach with bowel rest and pain management. Your clinician will assess severity, imaging findings, and individual risk factors to guide this decision.

When does diverticulitis require hospitalization or surgery?

Complicated diverticulitis — involving abscess, perforation, fistula, or obstruction — typically requires hospitalization with IV antibiotics and possibly surgical intervention. Recurrent episodes causing persistent symptoms or complications are also indications for elective surgical resection after the acute episode resolves.

What diet should I follow after a diverticulitis attack?

During an acute attack, a clear liquid diet transitioning to low-fiber as symptoms improve is standard. After recovery, a high-fiber diet is recommended to reduce recurrence risk — the evidence against seeds and nuts causing flares is weak and dietary restriction of these foods is no longer routinely advised.

What side effects are associated with diverticulitis treatments?

Oral antibiotics (metronidazole plus ciprofloxacin or amoxicillin-clavulanate) commonly cause GI upset, metallic taste, and diarrhea. IV antibiotics in hospitalized patients have broader monitoring considerations. Surgical treatment for complicated disease carries standard procedural risks including infection, anastomotic leak, and stoma formation.
