What is Diltiazem (Tiazac)?

Tiazac is an extended-release formulation of diltiazem, a calcium channel blocker used to treat high blood pressure, chronic stable angina, and certain heart rhythm disorders including atrial fibrillation and atrial flutter. It works by blocking calcium channels in both vascular smooth muscle and cardiac muscle cells—relaxing blood vessels to lower blood pressure and slowing the electrical conduction through the heart to control rate. Unlike dihydropyridine calcium channel blockers like amlodipine, diltiazem acts on both the heart and blood vessels, making it effective for both blood pressure and heart rate management. It's taken once daily.

Dual cardiac and vascular calcium blockade

Blocks calcium channels in both blood vessel walls and cardiac conduction tissue—relaxing peripheral blood vessels to reduce blood pressure while also slowing AV node conduction to control ventricular rate in atrial fibrillation.

Heart rate and rhythm control

By slowing electrical conduction through the AV node, Diltiazem controls the rapid ventricular rate associated with atrial fibrillation and flutter—reducing palpitations, improving cardiac efficiency, and protecting the heart from rate-related damage.

Extended-release once-daily control

Tiazac's extended-release formulation delivers diltiazem consistently over 24 hours, providing smooth blood pressure and heart rate control without the variability of shorter-acting formulations and with a simplified once-daily dosing schedule.

Frequently Asked Questions

What conditions is Tiazac used for?

Tiazac is an extended-release diltiazem used to treat high blood pressure, chronic stable angina, and to control heart rate in atrial fibrillation and flutter — conditions where both blood pressure and rate control are needed simultaneously.

How is diltiazem different from amlodipine for blood pressure?

Unlike amlodipine, which acts almost exclusively on blood vessels, diltiazem also slows electrical conduction through the heart — providing rate control alongside blood pressure reduction. This makes it particularly useful when both are needed.

Can I stop Tiazac suddenly?

No — particularly for people using it for angina or heart rate control. Abrupt discontinuation can worsen angina and cause rebound heart rate increases. Always taper gradually under your clinician's supervision.

What side effects might I experience?

Dizziness, headache, fatigue, slow heart rate, ankle swelling, and constipation are most common. Because diltiazem also slows cardiac conduction, it should not be used with other medications that have the same effect without clinician guidance.

Does diltiazem interact with other medications?

Yes — significantly. Diltiazem inhibits CYP3A4 and interacts with statins, cyclosporine, digoxin, and many other medications. A thorough medication review before starting is essential to prevent dangerous interactions.
