Antibiotic prophylaxis before certain dental procedures is recommended for patients at high risk of infective endocarditis — a serious heart valve infection. Dental procedures that manipulate gingival tissue or the periapical region of teeth can introduce bacteria into the bloodstream, posing risk for vulnerable individuals.

Who currently qualifies for dental antibiotic prophylaxis?

Current AHA/ACC guidelines limit prophylaxis to patients with the highest endocarditis risk: prosthetic cardiac valves or valve repair material, previous infective endocarditis, certain congenital heart defects, and cardiac transplant recipients with valve disease. Prophylaxis is no longer recommended for mitral valve prolapse or most other cardiac conditions.

Which dental procedures require prophylaxis?

Prophylaxis is recommended for procedures involving manipulation of gingival tissue, the periapical region of teeth, or perforation of the oral mucosa — including extractions, periodontal procedures, implant placement, and root canals. Routine dental X-rays, fillings on non-inflamed tissue, and orthodontic adjustments generally do not require prophylaxis.

What antibiotic is used for dental prophylaxis and when is it taken?

Amoxicillin 2g orally is the standard regimen — taken 30–60 minutes before the dental procedure. For penicillin-allergic patients, alternatives include clindamycin, cephalexin, azithromycin, or clarithromycin depending on the allergy type and severity.

What side effects are associated with dental prophylaxis antibiotics?

A single dose of amoxicillin is generally very well tolerated — GI upset and rash are the most common effects. Allergic reactions range from mild rash to anaphylaxis in penicillin-allergic individuals. Clindamycin alternatives carry GI side effect and — with single doses — a very low Clostridioides difficile risk.
