What is Cyproheptadine?

Cyproheptadine is a first-generation antihistamine with a broader range of uses than most allergy medications. While it treats allergic symptoms like itching, hives, and runny nose, it's also widely used off-label to stimulate appetite in patients with poor appetite or unintentional weight loss, to prevent migraines, and to manage serotonin syndrome. Its appetite-stimulating effect comes from its blockade of serotonin receptors involved in satiety signaling. It causes significant sedation, which can be a side effect or a therapeutic benefit depending on the patient's needs.

Histamine and serotonin blockade

Blocks both H1 histamine receptors and serotonin receptors, giving it a broader pharmacological profile than standard antihistamines—accounting for its effectiveness across allergy, appetite, and migraine applications.

Appetite stimulation

By blocking serotonin receptors involved in satiety, Cyproheptadine reduces the brain signals that suppress hunger—making it a useful off-label option for patients with poor appetite, low weight, or failure to thrive.

Migraine prevention

Used at low daily doses to reduce migraine frequency, particularly in pediatric and adolescent patients, where it has a well-established off-label track record for prophylactic headache management.

Frequently Asked Questions

What conditions is cyproheptadine used for?

Cyproheptadine is used for allergic symptoms, appetite stimulation in people with unintentional weight loss, and migraine prevention — particularly in children and adolescents. Your prescribed use will depend on your specific situation.

How does it stimulate appetite?

By blocking serotonin receptors involved in satiety signaling, cyproheptadine reduces the brain's sense of fullness — increasing hunger and food intake. This effect is most useful for illness-related appetite loss.

How long before it helps with migraines?

For migraine prevention, meaningful reduction in attack frequency typically takes 4–8 weeks of consistent daily use. It's most commonly used in pediatric patients where other preventives may not be appropriate.

What side effects might I experience?

Drowsiness is the most prominent side effect — it can be significant, particularly early in treatment. Dry mouth, dizziness, increased appetite, and weight gain are also common. The sedation often improves over time.

Can cyproheptadine be used in children?

Yes. Cyproheptadine is one of the more established medications for migraine prevention in children and adolescents. It's also used for appetite stimulation in pediatric patients with failure to thrive.
