What is Corlanor (Ivabradine)?

Corlanor is a prescription medication used to reduce the risk of hospitalization in adults with stable, symptomatic heart failure with reduced ejection fraction whose resting heart rate is 70 beats per minute or higher, and who are already on maximum tolerated beta-blocker therapy. It works by selectively blocking the funny current (If) in the sinoatrial node—the heart's natural pacemaker—slowing heart rate without affecting blood pressure or the strength of cardiac contractions. By reducing heart rate alone, it decreases myocardial oxygen demand and gives the heart more time to fill with each beat.

Selective If channel blockade

Blocks the hyperpolarization-activated cyclic nucleotide-gated (HCN) channels in the sinoatrial node that generate the funny current—slowing the pacemaker firing rate and reducing heart rate without affecting cardiac contractility or vascular tone.

Heart rate reduction without hypotension

Unlike beta-blockers, Ivabradine lowers heart rate through a purely pacemaker-specific mechanism—with no effect on blood pressure, making it valuable when further beta-blocker dose increases are limited by hypotension.

Heart failure hospitalization reduction

By slowing heart rate, Corlanor reduces myocardial oxygen demand and allows longer ventricular filling time—improving cardiac efficiency and reducing the risk of worsening heart failure and hospitalization in appropriately selected patients.

No. Corlanor is specifically indicated for patients in normal sinus rhythm. It should not be used in patients with atrial fibrillation or other arrhythmias where the sinoatrial node isn't the primary cardiac pacemaker.
