What is Coreg (Carvedilol)?

Coreg is a non-selective beta-blocker with additional alpha-1 blocking activity, used to treat high blood pressure, heart failure with reduced ejection fraction, and left ventricular dysfunction following a heart attack. Its dual mechanism—blocking both beta and alpha-1 adrenergic receptors—means it reduces heart rate and cardiac workload while also causing vasodilation, providing more comprehensive cardiovascular effects than beta-1 selective blockers alone. Coreg is one of only a few medications proven to reduce mortality in heart failure, making it a cornerstone of heart failure therapy. It's taken twice daily with food.

Non-selective beta and alpha-1 blockade

Blocks beta-1 and beta-2 adrenergic receptors to slow heart rate and reduce contractility, while simultaneously blocking alpha-1 receptors to cause vasodilation—lowering both cardiac output and peripheral vascular resistance for comprehensive blood pressure and cardiac load reduction.

Proven heart failure mortality reduction

One of three beta-blockers with robust clinical trial evidence showing reduced mortality in heart failure with reduced ejection fraction—making Carvedilol a guideline-directed, evidence-based cornerstone of heart failure management.

Post-MI cardiac protection

Reduces the risk of cardiovascular death and cardiac remodeling after myocardial infarction by blocking the harmful neurohormonal activation that drives progressive ventricular dysfunction in the weeks and months following a heart attack.

Frequently Asked Questions

What conditions is Coreg used for?

Coreg is used for high blood pressure, heart failure with reduced ejection fraction, and left ventricular dysfunction following a heart attack. It's one of only three beta-blockers with proven mortality benefit in heart failure.

How is Coreg different from other beta-blockers?

Carvedilol blocks both beta-1 and beta-2 adrenergic receptors as well as alpha-1 receptors — causing vasodilation that most other beta-blockers don't provide. This dual mechanism reduces both heart rate and peripheral vascular resistance.

Why must Coreg be taken with food?

Taking carvedilol with food slows its absorption and significantly reduces the risk of orthostatic hypotension — dizziness or lightheadedness upon standing — particularly important when starting treatment or increasing the dose.

What side effects might I experience?

Dizziness, fatigue, slow heart rate, low blood pressure upon standing, weight gain from fluid retention, and cold extremities are most common. Mask hypoglycemia symptoms in diabetic patients — blood sugar monitoring is important.

Can I stop Coreg suddenly?

No — particularly for patients with heart disease. Abrupt discontinuation can trigger rebound tachycardia, worsening angina, and potentially serious cardiac events. Always taper gradually over 1–2 weeks under your clinician's guidance.
