What is a CGM and how does it work?

A continuous glucose monitor uses a small sensor inserted just under the skin — typically on the arm or abdomen — to measure interstitial glucose levels every few minutes. Real-time readings, trend arrows, and customizable alerts are displayed on a smartphone or receiver, providing far more information than intermittent finger-stick testing.

Who is a CGM appropriate for?

CGMs are most established for type 1 diabetes and insulin-using type 2 patients. Coverage has expanded significantly, and evidence now supports CGM use in non-insulin-using type 2 patients for motivation, behavioral feedback, and identifying postprandial patterns. Your clinician will advise whether CGM fits your management plan and insurance coverage.

Does a CGM replace finger-stick testing entirely?

For most users, CGM largely or completely replaces routine finger-stick testing. Calibration checks or confirmatory finger sticks are occasionally needed — particularly when readings don't match symptoms, during rapid glucose changes, or when making critical treatment decisions. CGM readings slightly lag behind blood glucose, which is clinically relevant in some situations.

How accurate are CGMs?

Modern CGMs (Dexcom G7, Libre 3, Eversense) have accuracy comparable to laboratory blood glucose meters for most glucose ranges, with mean absolute relative differences (MARD) of 8–10%. Accuracy is slightly reduced at hypoglycemic ranges, during rapid glucose changes, and in the first day after sensor insertion.

What side effects are associated with CGM use?

Sensor insertion causes brief, mild discomfort. Adhesive skin reactions — ranging from mild irritation to allergic contact dermatitis — are the most common ongoing concern and can be managed with skin barrier products or by changing sensor locations. Sensor failures requiring early replacement are occasionally reported. No serious systemic risks are associated with CGM use.
