What features make a mole concerning?

The ABCDE criteria guide evaluation: Asymmetry (one half doesn't match the other), Border irregularity (ragged, notched, or blurred edges), Color variation (multiple shades of brown, black, red, or white), Diameter greater than 6mm, and Evolution (any change in size, shape, color, or symptoms). Any change or new symptom warrants clinical evaluation.

Does a concerning mole mean I have melanoma?

Not necessarily — the majority of biopsied concerning moles are benign, including dysplastic (atypical) nevi, seborrheic keratoses, and other benign growths. Biopsy is the only way to definitively determine whether a lesion is benign, atypical, or malignant. Early evaluation provides the best outcomes if melanoma is present.

What happens during a mole biopsy?

A clinician uses a sterile technique under local anesthesia to remove part or all of the lesion — shave biopsy, punch biopsy, or excisional biopsy depending on the clinical presentation. The specimen is sent to pathology for examination. Results are typically available within 1–2 weeks.

If a mole is found to be atypical (dysplastic), what happens next?

Dysplastic nevi are graded mild, moderate, or severe. Mild dysplasia is usually managed with routine monitoring. Moderate to severe dysplasia typically requires a wider re-excision to ensure clear margins, as these lesions have a small but real risk of progression. Your clinician will explain the specific pathology findings and recommended next steps.

What side effects are associated with mole evaluation and treatment?

Biopsy under local anesthesia causes brief discomfort and leaves a small scar — size varies by biopsy method. Wide excision for atypical or malignant lesions leaves a larger scar proportional to the excision size. Infection and wound healing issues are uncommon.
