What is a Comprehensive Testosterone Panel?

The Comprehensive Testosterone Panel measures total testosterone, free testosterone, and bioavailable testosterone — providing a complete picture of testosterone status rather than a single number. Because much of testosterone is bound to proteins and biologically inactive, measuring all three fractions is the only way to understand how much testosterone your body actually has available to use.

Total testosterone

Measures all testosterone in the blood — free, albumin-bound, and SHBG-bound. While a useful starting point, total testosterone alone can be misleading when protein binding is elevated, making it necessary to interpret alongside the other fractions.

Free testosterone — the active fraction

The unbound portion of testosterone that is immediately available to tissues. Low free testosterone despite normal total testosterone is a common and frequently missed cause of fatigue, low libido, and reduced muscle function in both men and women.

Bioavailable testosterone — the full functional picture

Combines free testosterone with the loosely albumin-bound portion, representing everything the body can readily access. This is often the most clinically useful number for evaluating symptoms and guiding treatment decisions.

Free Testosterone

This is the testosterone not bound to proteins in your blood—it's considered the biologically active portion.

Bioavailable Testosterone

Includes free testosterone plus the testosterone loosely bound to albumin, representing what’s readily available to the body.

Total Testosterone

Encompasses all forms of testosterone in your blood, including free, albumin-bound, and SHBG-bound.

Frequently Asked Questions

What does the Comprehensive Testosterone Panel include?

The panel measures total testosterone, free testosterone, and SHBG (sex hormone-binding globulin). Together these markers give a complete picture of testosterone status — because SHBG affects how much testosterone is biologically active, total testosterone alone can sometimes be misleading.

Why does SHBG matter?

SHBG binds testosterone in the bloodstream, making it unavailable for use. If SHBG is high, free testosterone (the active portion) may be low even when total testosterone is normal — explaining symptoms that standard testing would miss. This panel captures that nuance.

When should this test be done?

Testosterone levels peak in the morning, so testing between 7–10 AM gives the most clinically relevant result. Levels decline by 20–30% through the day. Fasting is not strictly required but morning testing is strongly recommended.

Is this test appropriate for women?

Yes — testosterone plays an important role in female health, including energy, sexual function, muscle maintenance, and mood. Women with unexplained fatigue, low libido, or poor response to other hormonal treatments may benefit from a comprehensive testosterone assessment.

What if my free testosterone is low but my total testosterone looks normal?

This is exactly what this panel is designed to detect. Low free testosterone despite normal total testosterone — often due to elevated SHBG — can explain symptoms of low testosterone. Treatment decisions and further evaluation are based on the full panel, not total testosterone alone.
