What is a Comprehensive Fatigue Panel?

A Comprehensive Fatigue Panel is a broad blood and urine test designed to systematically identify the most common medical causes of persistent tiredness. It combines 76 biomarkers across multiple body systems — from thyroid and adrenal function to inflammation, autoimmune markers, and viral infections — to find what standard panels often miss.

Hormonal, metabolic, and blood health

Evaluates thyroid function (TSH, Free T4), morning cortisol, iron stores, CBC, and HbA1c to identify anemia, thyroid dysfunction, adrenal issues, and blood sugar problems that commonly drive fatigue.

Inflammation, autoimmune, and viral causes

Measures CRP, ESR, ANA, EBV antibodies, and screens for celiac disease and HIV — addressing immune and inflammatory contributors that are frequently overlooked in routine fatigue workups.

Nutritional status and organ function

Assesses Vitamin B12, folate, Vitamin D, and a comprehensive metabolic panel covering kidney, liver, and electrolyte function to catch deficiencies and organ-related causes of low energy.

1 to 5 days

CBC (Complete Blood Count)

Assesses overall blood health and detects conditions like anemia, infection, and immune disorders.

Includes: White Blood Cells (WBC), Red Blood Cells (RBC), Hemoglobin (Hgb), Platelet Count, Differential Counts

CMP (Comprehensive Metabolic Panel)

Evaluates kidney and liver function, blood sugar levels, and electrolyte balance.

Includes: Albumin, Calcium, Glucose, BUN/Creatinine Ratio, Liver Enzymes (ALT, AST), Electrolytes

Thyroid Panel

Evaluates metabolism and hormonal balance—key factors in fatigue.

Includes: TSH, Free T4 (FT4)

Hemoglobin A1c (HbA1c)

Reveals average blood sugar over 3 months to rule out diabetes or prediabetes.

Iron Panel

Evaluates iron levels and storage to assess for anemia or other iron-related conditions. Ferritin also serves as an inflammatory marker that can be elevated in infection, autoimmune conditions, or chronic disease.

Includes: Total Iron, Ferritin, Iron Binding Capacity, Transferrin Saturation

Inflammatory Markers

Detects inflammation, which may indicate infection, autoimmune disorders, or chronic disease.

Includes: C-Reactive Protein (CRP), Erythrocyte Sedimentation Rate (ESR)

Morning Cortisol

Measures cortisol levels to evaluate adrenal function and stress response.

Creatine Kinase (CK)

Assesses muscle health and detects muscle damage or disorders.

Vitamin Panel

Evaluates key vitamin levels that impact energy, nerve function, and overall wellness.

Includes: Vitamin B12, Folate, Vitamin D

HIV Screening

Detects HIV infection, which can impact immune function and energy levels.

EBV Panel (Epstein-Barr Virus)

Checks for Epstein-Barr virus, a common cause of chronic fatigue syndrome.

Autoimmune Screen (ANA)

Screens for autoimmune diseases that may contribute to chronic fatigue.

Celiac Disease Panel

Screens for celiac disease, which can cause fatigue due to nutrient malabsorption.

Frequently Asked Questions

What conditions can this panel detect?

The panel screens for the most common medical causes of persistent tiredness — including anemia, iron deficiency, thyroid dysfunction, prediabetes and diabetes, vitamin D deficiency, and liver or kidney issues. It measures 76 biomarkers across multiple body systems in a single blood draw.

Do I need to fast before this test?

Yes — fasting for 8–12 hours before your blood draw is recommended for the most accurate results, particularly for the blood sugar and metabolic markers included in the panel. Water is fine. Schedule your appointment for the morning if possible.

What if my results come back normal but I still feel exhausted?

Normal results are still valuable — they help rule out common correctable causes. A clinician can review your results alongside your symptoms and health history to determine whether further evaluation, specialist referral, or a different approach is warranted.

Is this test appropriate for both men and women?

Yes — the panel is designed for both. It includes hormones relevant to each, including testosterone, thyroid hormones, cortisol, and DHEA-S, alongside universal markers like iron stores, blood counts, and metabolic function.

How soon will I have results?

Results are typically available within 2–4 business days. A General Medicine clinician will review your results and can recommend next steps or book a same-day visit if further evaluation is needed.
