At what age should I have my first colonoscopy?

Current guidelines recommend colorectal cancer screening beginning at age 45 for average-risk individuals. People with a first-degree relative diagnosed with colorectal cancer or advanced polyps before age 60 should begin screening at age 40, or 10 years before the relative's diagnosis — whichever comes first.

Is colonoscopy the only colorectal cancer screening option?

No — alternatives include stool-based tests (FIT annually, Cologuard every 3 years) and CT colonography every 5 years. Colonoscopy remains the most comprehensive option because it both detects and removes polyps in a single procedure. Abnormal results from other tests require follow-up colonoscopy.

How do I prepare for a colonoscopy?

Bowel preparation — drinking a laxative solution the day before — is required for an adequate exam. Low-fiber diet for 1–3 days prior is also recommended. The preparation is often cited as the least comfortable part of the process. You'll need a driver for the day of the procedure due to sedation.

How long does the procedure take and will it be painful?

The colonoscopy itself takes 20–45 minutes. Patients receive sedation — most have no memory of the procedure and report minimal discomfort. Mild bloating or cramping afterward is common and resolves within hours.

What risks are associated with colonoscopy?

Colonoscopy is very safe — serious complications are uncommon. Perforation occurs in less than 1 in 1,000 procedures. Bleeding after polyp removal is the most common complication, typically resolving without intervention. Post-procedure gas and bloating are expected and temporary.
