What is chronic rhinosinusitis and how is it diagnosed?

Chronic rhinosinusitis (CRS) is inflammation of the sinuses lasting 12 weeks or more despite treatment, characterized by nasal congestion, facial pressure, postnasal drip, and reduced sense of smell. Diagnosis is clinical — confirmed with nasal endoscopy or CT imaging when presentation is atypical or surgery is considered.

What is the difference between CRS with and without nasal polyps?

CRS with nasal polyps (CRSwNP) involves benign growths in the sinus and nasal passages — more commonly associated with asthma and aspirin sensitivity. CRS without polyps (CRSsNP) is more often associated with infection or anatomical factors. Treatment strategies, particularly for severe cases, differ between the two subtypes.

When is surgery considered for chronic sinusitis?

Functional endoscopic sinus surgery (FESS) is considered when symptoms significantly impact quality of life despite adequate medical management — including nasal corticosteroids, saline rinses, and appropriate antibiotics. Surgery improves drainage and access for topical treatment but does not address the underlying inflammatory process.

Does having chronic sinusitis increase my cancer risk?

CRS itself is not a cancer risk factor. Persistent nasal symptoms — particularly unilateral symptoms, bleeding, or facial numbness — that don't respond to standard treatment warrant evaluation to rule out other causes including rare sinus tumors. This is distinct from typical bilateral CRS.

What side effects are associated with chronic rhinosinusitis treatments?

Nasal corticosteroid sprays are very well tolerated — occasional nosebleeds and mild nasal irritation may occur. Long-term oral steroids carry significant systemic effects and are avoided. Dupilumab (for CRSwNP) may cause injection site reactions and conjunctivitis. Antibiotics carry standard GI and resistance considerations.
