Chronic daily headache is defined as headaches occurring 15 or more days per month for at least 3 months. Chronic migraine — the most common subtype — involves 8 or more migraine-quality days monthly. Reaching this threshold warrants a clinical evaluation and discussion of preventive treatment.

Could my frequent headaches be caused by the medication I take for them?

Yes — medication overuse headache (MOH) is one of the most common and underrecognized causes of chronic daily headache. Using acute treatments — triptans, NSAIDs, or even OTC pain relievers — more than 10–15 days per month can perpetuate and worsen headache frequency.

What preventive treatments are available for chronic headache?

Options include daily oral medications — topiramate, amitriptyline, beta-blockers, valproate — as well as monthly injectable CGRP monoclonal antibodies, which are specifically indicated for chronic migraine and have strong evidence for reducing headache days.

Should I have imaging done for my chronic headaches?

Imaging is not routinely indicated for chronic headaches with a consistent pattern and no neurological symptoms. Red flags — new or sudden onset, progressive worsening, neurological deficits, or headaches waking from sleep — warrant imaging to exclude secondary causes.

What side effects are associated with chronic headache treatments?

Topiramate may cause cognitive slowing and word-finding difficulty. Amitriptyline causes drowsiness and dry mouth. Beta-blockers may cause fatigue.
