The most common causes are upper airway cough syndrome (post-nasal drip), asthma, and gastroesophageal reflux disease — which together account for the majority of chronic cough cases. ACE inhibitor medication is another important and easily reversible cause.

Could my blood pressure medication be causing my cough?

Yes — ACE inhibitors cause a dry, persistent cough in 5–15% of patients due to bradykinin accumulation. This is one of the most important and easily missed causes of chronic cough. Switching to an ARB (a related class without this side effect) typically resolves the cough within 1–4 weeks.

When should chronic cough prompt further investigation beyond common causes?

Imaging and further workup are appropriate when common causes have been adequately treated without resolution, when the cough is associated with hemoptysis, significant weight loss, hoarseness, or dysphagia, or in patients with significant smoking history or immunosuppression where malignancy is a concern.

Does treating reflux help with chronic cough?

In patients with cough-variant GERD — where reflux triggers cough without heartburn — aggressive acid suppression with a PPI trial over 8 weeks can confirm the diagnosis and provide relief. Lifestyle measures (elevating the head of the bed, avoiding late meals) should accompany medication.

What side effects are associated with chronic cough treatments?

Intranasal steroids and antihistamines for upper airway cough syndrome are well tolerated. PPIs for reflux-related cough may affect magnesium and B12 levels with prolonged use. Inhaled bronchodilators and steroids for cough-variant asthma carry standard inhaler side effect profiles. Gabapentinoids for refractory cough may cause sedation.
