What is cellulitis and how do I recognize it?

Cellulitis is a bacterial infection of the deeper layers of the skin — presenting as an area of spreading redness, warmth, swelling, and tenderness, most commonly on the lower legs. It typically has poorly defined borders and no central fluctuance (pus pocket) — distinguishing it from an abscess.

When does cellulitis need emergency evaluation?

Seek urgent or emergency evaluation for rapidly spreading redness, high fever, chills, red streaking (lymphangitis), significant swelling of an entire limb, or any cellulitis in an immunocompromised patient or diabetic — these features suggest severe infection or, rarely, necrotizing fasciitis requiring immediate treatment.

How long does cellulitis take to improve with antibiotics?

Improvement — reduced redness, less warmth, decreased swelling — is typically seen within 24–48 hours of starting appropriate antibiotics. Marking the borders of the redness with a pen at diagnosis helps track whether it is spreading or receding. The full antibiotic course (typically 5–7 days) must be completed.

Why do I keep getting cellulitis in the same area?

Recurrent cellulitis in the same site often reflects a persistent entry point — tinea pedis (athlete's foot) between the toes is a very common and frequently overlooked source for lower leg cellulitis. Chronic lymphedema, venous insufficiency, and obesity also predispose to recurrence. Treating the underlying cause reduces recurrence significantly.

What side effects are associated with cellulitis treatments?

Oral antibiotics (cephalexin, dicloxacillin, or TMP-SMX when MRSA is suspected) commonly cause GI upset. IV antibiotics in hospitalized patients carry broader monitoring requirements. Elevating the affected limb is important and carries no side effects. Compression therapy after resolution supports venous return in recurrence-prone patients.
