What is carpal tunnel syndrome and what causes it?

Carpal tunnel syndrome (CTS) results from compression of the median nerve as it passes through the carpal tunnel in the wrist — causing numbness, tingling, and weakness in the thumb, index, middle, and radial half of the ring finger. Contributing factors include repetitive hand use, wrist anatomy, pregnancy, thyroid disease, and diabetes.

How is carpal tunnel syndrome diagnosed?

Diagnosis is primarily clinical — based on characteristic symptom distribution, positive Phalen's and Tinel's signs on exam, and symptom pattern (worse at night, with phone use, or driving). Nerve conduction studies confirm the diagnosis and quantify severity when clinical assessment is unclear or surgery is being considered.

Can carpal tunnel syndrome improve without surgery?

Yes — many cases improve significantly with conservative treatment. Nighttime wrist splinting in neutral position is the most evidence-supported non-surgical intervention. Corticosteroid injection provides medium-term relief. Activity modification, physical therapy, and treating underlying conditions (hypothyroidism, diabetes) also contribute.

When is surgery recommended for carpal tunnel syndrome?

Surgery is recommended when conservative treatment has failed after 2–7 weeks, when severe or progressive symptoms are present, or when significant thenar muscle weakness (thumb base muscle wasting) indicates risk of permanent nerve damage. Carpal tunnel release has a high success rate and short recovery time.

What side effects are associated with carpal tunnel treatments?

Wrist splints are very safe — occasional skin irritation from straps. Corticosteroid injections may cause temporary pain flare, skin thinning, and tendon weakening with repeated injections. Surgical release carries risks of infection, scar tenderness, and incomplete relief — serious complications including nerve or tendon injury are uncommon.
