What causes canker sores?

Canker sores (aphthous ulcers) result from a combination of local trauma, stress, nutritional deficiencies (B12, folate, iron, zinc), hormonal changes, food sensitivities (acidic foods, SLS in toothpaste), and immune dysregulation. They are not caused by the herpes virus and are not contagious.

How do canker sores differ from cold sores?

Canker sores appear inside the mouth — on the soft tissue of the cheeks, tongue, and inner lips — and are not infectious. Cold sores (herpes labialis) appear on the outer lip border, are caused by HSV-1, and are contagious. Canker sores tend to be rounder with a yellow-white center and red halo.

How long do canker sores last and when do they require evaluation?

Most canker sores resolve within 1–2 weeks without treatment. Evaluation is appropriate for sores persisting beyond 3 weeks, sores that are unusually large or painful, recurrent episodes limiting eating or speaking, or sores accompanied by fever, skin rash, or genital ulcers — which may suggest systemic conditions.

What can I do to reduce the pain and speed healing?

OTC topical treatments — benzocaine gel, hydrogen peroxide rinse, or sucralfate paste — reduce pain. Prescription triamcinolone dental paste or topical dexamethasone can accelerate healing for severe or frequent ulcers. Avoiding spicy, acidic, and abrasive foods during an outbreak reduces irritation.

What side effects are associated with canker sore treatments?

Topical benzocaine is well tolerated — methemoglobinemia is a very rare concern with excessive use. Topical corticosteroids may cause local thinning with very prolonged use. OTC oral rinses are very safe. Identifying and addressing nutritional deficiencies or dietary triggers carries no side effects and may prevent recurrence.
