Burning mouth syndrome (BMS) is a chronic condition characterized by persistent burning, scalding, or tingling sensation in the mouth — most commonly affecting the tongue, lips, and palate — without an identifiable clinical or laboratory abnormality. It primarily affects perimenopausal and postmenopausal women.

What causes burning mouth syndrome?

Primary BMS involves altered pain processing — a neuropathic condition. Secondary BMS can be caused by nutritional deficiencies (B12, folate, iron, zinc), dry mouth, denture irritation, oral thrush, hormonal changes, or certain medications. A clinical evaluation distinguishes primary from secondary causes.

Can nutritional deficiencies cause a burning mouth sensation?

Yes — deficiencies in B12, iron, folate, and zinc are among the most important and treatable secondary causes of oral burning. Blood testing for these nutrients is a routine part of BMS evaluation. Correcting identified deficiencies produces significant symptom improvement in secondary BMS.

What treatments are available for primary burning mouth syndrome?

Treatment is challenging and often requires a trial-and-error approach. Options with supporting evidence include low-dose clonazepam (used as an oral rinse or swallowed), alpha-lipoic acid, low-dose antidepressants, gabapentinoids, and cognitive behavioral therapy. Symptom management rather than cure is the realistic goal.

What side effects are associated with burning mouth syndrome treatments?

Clonazepam carries sedation and dependency risk — topical use as an oral rinse (spit rather than swallow) reduces systemic effects. Low-dose TCAs cause dry mouth and drowsiness. Gabapentinoids may cause dizziness and sedation. Alpha-lipoic acid is very well tolerated. CBT has no physical side effects.
