What is Brenzavvy (Bexagliflozin)?

Brenzavvy is a prescription SGLT2 inhibitor used to improve blood sugar control in adults with Type 2 diabetes. Like other medications in its class, it works by blocking the SGLT2 transporter in the kidneys, which normally reabsorbs glucose back into the bloodstream—causing the kidneys to excrete excess glucose in the urine instead. This lowers blood sugar without stimulating insulin and typically produces modest weight loss and mild blood pressure reduction as additional benefits. It's taken once daily and should not be used in patients with severe kidney impairment, as reduced kidney function limits its glucose-lowering efficacy.

SGLT2 transporter inhibition

Blocks the sodium-glucose cotransporter 2 in the kidney's proximal tubule, preventing reabsorption of filtered glucose and causing the kidneys to excrete approximately 60–80 grams of glucose per day in the urine—lowering blood sugar independent of insulin.

Insulin-independent glucose lowering

Because it works through renal glucose excretion rather than insulin pathways, Bexagliflozin lowers blood sugar without stimulating insulin release—carrying a low intrinsic risk of hypoglycemia when used without insulin or sulfonylureas.

Cardiometabolic secondary benefits

Glucose excreted in urine represents lost calories, contributing to modest weight loss; simultaneous osmotic diuresis produces mild blood pressure lowering—additional metabolic benefits relevant to the cardiovascular risk profile of Type 2 diabetes patients.

Frequently Asked Questions

How does Brenzavvy lower blood sugar?

Brenzavvy blocks the SGLT2 transporter in the kidneys — preventing reabsorption of filtered glucose and causing excess sugar to be excreted in the urine rather than returned to the bloodstream.

Can Brenzavvy cause low blood sugar on its own?

Brenzavvy alone has a low risk of hypoglycemia because it works independent of insulin — it simply removes glucose through urine regardless of insulin levels. Risk increases when combined with insulin or sulfonylureas.

Are there heart or kidney benefits with Brenzavvy like with other SGLT2 inhibitors?

Bexagliflozin is a newer SGLT2 inhibitor — long-term cardiovascular and kidney outcome trial data is still emerging compared to more established agents like empagliflozin and dapagliflozin. Discuss evidence specifics with your clinician.

What side effects might I experience?

Genital yeast infections and urinary tract infections are the most distinctive side effects — due to increased glucose in the urine creating a favorable environment for microbial growth. Dehydration and low blood pressure can occur in susceptible patients.

Should I stay well hydrated while taking Brenzavvy?

Yes. The increased urinary glucose excretion draws water with it — adequate hydration is important to prevent dehydration-related kidney effects, particularly during illness, hot weather, or vigorous exercise.
