What causes body odor?

Body odor primarily results from bacteria on the skin breaking down sweat compounds — particularly apocrine gland secretions in the underarms and groin. Diet (garlic, onions, certain spices), medications, hormonal changes, and underlying medical conditions can influence odor quality and intensity.

When should body odor prompt a medical evaluation?

Sudden change in body odor, odor that doesn't respond to hygiene measures, or odor described as fruity (possible diabetes), fishy (possible trimethylaminuria or bacterial vaginosis), or ammonia-like (possible kidney disease) warrants clinical evaluation to rule out underlying systemic causes.

What treatments are available beyond OTC antiperspirants?

Prescription-strength aluminum chloride antiperspirants provide stronger effect. Prescription glycopyrronium cloth wipes (Qbrexza) are FDA-approved for axillary hyperhidrosis and odor. Botulinum toxin injections reduce sweating significantly for 4–12 months in the underarms.

Can diet changes reduce body odor?

Yes — reducing intake of foods like garlic, onions, red meat, and cruciferous vegetables can meaningfully reduce body odor in some people. Increasing magnesium and chlorophyll-rich foods is sometimes suggested, though evidence is limited. Staying well hydrated also dilutes sweat compounds.

What side effects are associated with body odor treatments?

Prescription aluminum chloride antiperspirants may cause skin irritation — applying to completely dry skin minimizes this. Glycopyrronium wipes may cause dry mouth and urinary hesitancy from systemic absorption. Botulinum toxin injections at the underarm site are very well tolerated with minimal systemic effects.
