What are the most common causes of bad breath?

In 85–90% of cases, bad breath originates in the mouth — from bacterial breakdown of food debris on the tongue, between teeth, and in gum pockets. Gum disease, dry mouth, poorly fitting dental work, and tonsil stones are other common oral sources. Systemic causes (GERD, sinus disease, kidney or liver disease) account for a minority of cases.

If I brush and floss regularly, why do I still have bad breath?

The tongue is the most overlooked source — bacterial coatings on the dorsal tongue produce volatile sulfur compounds that cause odor. Tongue scraping, in addition to brushing and flossing, significantly reduces oral odor. Dry mouth — from mouth breathing, medications, or Sjögren's — also causes persistent bad breath despite good hygiene.

When should bad breath prompt a medical evaluation?

When bad breath persists despite thorough oral hygiene, or is accompanied by a sweet or fruity smell (possible diabetes), ammonia smell (possible kidney disease), fecal odor (possible GI obstruction), or significant postnasal drip and nasal symptoms — a clinical evaluation is appropriate.

Can mouthwash cure bad breath?

Mouthwash provides temporary masking but doesn't address the underlying cause. Antibacterial mouthwashes (containing cetylpyridinium chloride or chlorhexidine) can reduce oral bacterial load — chlorhexidine is the most effective but causes tooth staining with prolonged use. Addressing the root cause produces lasting improvement.

What side effects are associated with bad breath treatments?

Chlorhexidine mouthwash causes tooth and tongue staining with regular use and may alter taste temporarily. Tongue scrapers and water flossers are very safe. Treating contributing GERD with acid suppression may cause the PPI side effects noted elsewhere. Dental treatment for gum disease involves standard procedural considerations.
