What is Azelex (azelaic acid)?

Azelex is a prescription topical cream containing 20% azelaic acid, used to treat mild to moderate acne. It works through multiple complementary mechanisms—killing acne-causing bacteria, normalizing the follicular keratinization that leads to clogged pores, and reducing the post-inflammatory hyperpigmentation that acne can leave behind. Unlike retinoids or benzoyl peroxide, azelaic acid is generally well-tolerated by sensitive skin and is safe during pregnancy, making it a useful option for patients who can't use other topical acne medications. It's applied twice daily to the entire affected area.

Antibacterial and antikeratinizing activity

Kills Cutibacterium acnes within follicles while simultaneously normalizing the abnormal keratinization that causes pores to clog—addressing two of the primary drivers of acne through a single ingredient.

Post-inflammatory hyperpigmentation reduction

Inhibits tyrosinase, the enzyme involved in melanin production, reducing the dark spots and uneven skin tone that frequently follow acne breakouts—providing both acne treatment and pigmentation correction.

Pregnancy-safe tolerability

One of the few topical acne treatments considered safe during pregnancy, and generally well-tolerated by sensitive or rosacea-prone skin—making it a valuable option when retinoids, benzoyl peroxide, or antibiotics aren't appropriate.

Frequently Asked Questions

What does Azelex treat?

Azelex is a prescription 20% azelaic acid cream used to treat mild to moderate acne. It reduces acne-causing bacteria, normalizes skin cell shedding to prevent pore clogging, and fades the dark spots that breakouts leave behind.

How long before Azelex clears my acne?

Most people see meaningful improvement after 4–8 weeks of consistent twice-daily use. Full results typically develop over 3–6 months. Azelex works gradually — it's not a fast-acting acne treatment.

Is Azelex safe for sensitive skin?

Yes — azelaic acid is one of the gentler topical acne treatments available, making it well suited for people with sensitive, rosacea-prone, or easily irritated skin who haven't tolerated retinoids or benzoyl peroxide.

What side effects might I experience?

Temporary burning, stinging, tingling, and itching at the application site are most common — especially in the first few weeks. Dryness and mild peeling can also occur. These typically improve as skin adjusts.

Is Azelex safe to use during pregnancy?

Azelaic acid is considered one of the safer topical acne treatments during pregnancy and is often recommended as an alternative to retinoids and certain antibiotics that are contraindicated in pregnancy. Confirm with your clinician.
