What is an anal fissure and what causes it?

An anal fissure is a small tear in the lining of the anal canal — most commonly caused by passing hard or large stools. It causes sharp pain during and after bowel movements, often with small amounts of bright red blood on toilet paper. Chronic fissures involve tightening of the internal anal sphincter, which impairs healing.

How is an anal fissure treated without surgery?

First-line treatment focuses on softening stools with increased fiber, fluid, and stool softeners, along with topical medications — nitrate cream or calcium channel blockers — to relax the internal sphincter and improve blood flow to the area. Sitz baths provide additional relief. Most acute fissures heal within 4–8 weeks.

Why do some anal fissures become chronic?

Chronic fissures result from a cycle where sphincter spasm reduces blood flow to the fissure, preventing healing — which perpetuates the spasm. Fissures present for more than 6–8 weeks are considered chronic and are less likely to resolve without topical sphincter-relaxing therapy or procedural intervention.

When is botulinum toxin or surgery considered?

Botulinum toxin injection into the sphincter is an office-based option for fissures that haven't responded to topical therapy — effective in 60–80% of cases. Lateral internal sphincterotomy (minor surgery) is more definitive with higher cure rates but carries a small risk of fecal incontinence.

What side effects are associated with anal fissure treatments?

Topical nitrate cream commonly causes headache due to systemic vasodilation — applying a small amount minimizes this. Calcium channel blocker cream has fewer systemic effects. Botulinum toxin injection may cause temporary incontinence in a small percentage of patients. Sphincterotomy carries a long-term incontinence risk of 1–5%.
