What is Amitriptyline?

Amitriptyline is a tricyclic antidepressant (TCA) used at higher doses to treat depression and at lower doses for a range of other conditions including chronic pain, neuropathic pain, migraine prevention, fibromyalgia, and insomnia. It works by blocking the reuptake of serotonin and norepinephrine while also blocking histamine and muscarinic receptors—producing both antidepressant and sedating effects. Despite being one of the older antidepressants, Amitriptyline remains widely used because of its effectiveness for pain conditions and its well-understood profile after decades of clinical use.

Serotonin and norepinephrine reuptake inhibition

Blocks reuptake transporters for both serotonin and norepinephrine, increasing their availability to regulate mood and modulate pain signaling in the central nervous system—relevant to both its antidepressant and analgesic effects.

Chronic pain and migraine prevention

At low doses (10–75 mg), Amitriptyline reduces central pain sensitization and inhibits the trigeminovascular activation that drives migraines—providing meaningful pain reduction for neuropathic pain, fibromyalgia, and migraine prevention independent of its antidepressant effect.

Sedating antihistamine activity

Potent H1 histamine receptor blockade produces significant sedation, making bedtime dosing helpful for patients with depression-related insomnia—while also contributing to side effects like daytime grogginess and weight gain with higher doses.
