What is the difference between an allergic reaction and anaphylaxis?

A mild allergic reaction involves localized symptoms — hives, itching, nasal congestion, or mild rash — without systemic involvement. Anaphylaxis is a severe, potentially life-threatening systemic reaction involving two or more body systems — skin, respiratory, cardiovascular, or GI — requiring immediate epinephrine and emergency care.

When should I use an epinephrine auto-injector?

Use epinephrine immediately if you experience throat tightness, difficulty breathing, significant wheezing, swallowing difficulty, dizziness, or rapid heartbeat after known allergen exposure — even before other symptoms develop. Epinephrine is the only first-line treatment for anaphylaxis — antihistamines and steroids are adjuncts, not substitutes.

What should I do after using an epinephrine auto-injector?

Call 911 or go immediately to the emergency department after using epinephrine — even if symptoms improve. Biphasic anaphylaxis (return of symptoms hours after initial treatment) occurs in up to 20% of cases. A minimum 4-hour observation period in a medical setting is recommended after anaphylaxis.

How do I identify what I'm allergic to?

Allergy testing — skin prick testing or specific IgE blood testing — identifies specific allergens when the trigger is not obvious from history. Oral food challenges are the gold standard for food allergy diagnosis. A clinical history of the reaction, timing, and suspected exposures guides which allergens to test.

What side effects are associated with allergic reaction treatments?

Epinephrine may cause palpitations, tremor, and anxiety — these are expected and brief. Diphenhydramine causes significant sedation. Oral corticosteroids at short courses cause insomnia, blood sugar elevation, and mood changes.
