What is Aldara (imiquimod)?

Aldara is a prescription topical cream used to treat external genital and perianal warts caused by HPV, actinic keratoses (precancerous skin lesions), and certain superficial basal cell carcinomas. Unlike treatments that physically destroy warts or lesions, imiquimod works by activating the body's own immune response at the application site—stimulating the skin's local defenses to recognize and attack abnormal tissue. It's applied directly to the affected area three times per week for up to 16 weeks depending on the condition, and is designed for patient self-administration at home.

Immune response modifier

Binds to toll-like receptor 7 (TLR7) on immune cells in the skin, triggering the innate immune system to produce interferon-alpha and other cytokines that activate local antiviral and antitumor immune responses.

HPV lesion clearance

By stimulating local immune activation, Aldara helps the immune system recognize and clear HPV-infected wart tissue—addressing the viral component of the infection rather than just physically removing the lesion.

Home-applied immunotherapy

Unlike office-based treatments such as cryotherapy or podophyllin, Aldara empowers patients to treat warts and actinic keratoses at home on a scheduled basis, providing a convenient alternative for appropriate lesions.

For external genital warts, treatment typically lasts up to 16 weeks. Actinic keratoses are usually treated for 16 weeks as well. Response varies — some people clear lesions sooner; others require the full course.

How often do I apply Aldara?

For genital warts, apply three times per week — not consecutive nights. For actinic keratoses, apply twice weekly. The schedule includes rest days to allow the skin to recover between applications.

Will the treated area look worse before it gets better?

Yes. Redness, erosion, flaking, and swelling in the treated area are signs the immune response is being activated — this is expected and indicates the treatment is working. These reactions typically resolve after treatment ends.

What side effects might I experience?

Local skin reactions — redness, itching, burning, erosion, and pain — are very common and expected. Flu-like symptoms including fatigue, headache, and body aches can occur as the immune system responds.

Can Aldara be used on warts inside the vagina or anus?

No. Aldara is approved for external genital and perianal warts only — not for warts on internal mucosal surfaces. Internal lesions require evaluation and treatment by a specialist using different approaches.
