What is Ajovy (Fremanezumab)?

Ajovy is a CGRP monoclonal antibody injection used for the preventive treatment of migraine in adults who experience either episodic or chronic migraines. It works by targeting calcitonin gene-related peptide (CGRP), a protein released during migraines that triggers the pain and inflammation of the attack. By neutralizing CGRP before it can bind to its receptors, Ajovy reduces both the frequency and severity of migraines over time. It can be injected once monthly or once quarterly depending on the dosing option chosen, making it one of the most flexible migraine preventives available.

CGRP neutralization

Binds directly to CGRP—the neuropeptide released during migraine attacks that causes vasodilation and neurogenic inflammation—preventing it from reaching its receptors and triggering the pain cascade.

Preventive monthly or quarterly dosing

Offers two dosing schedules—monthly injections or a higher quarterly dose—giving patients and providers flexibility to choose the frequency that best fits lifestyle and preference.

Episodic and chronic migraine coverage

Approved for both episodic (fewer than 15 headache days/month) and chronic migraine (15 or more), making it one of the few preventive treatments with evidence across both migraine classifications.

Frequently Asked Questions

How is Ajovy different from triptans for migraines?

Ajovy prevents migraines from occurring — it's not used during an attack. Triptans are taken acutely to stop an attack in progress. They serve completely different roles in migraine management and can be used together.

Can I choose between monthly and quarterly dosing?

Yes. Ajovy offers both dosing options — one injection monthly or a higher dose once every 3 months. Your clinician will help you decide based on your preference and clinical considerations.

How long before Ajovy reduces my migraine frequency?

Many people notice a reduction in migraine days within the first month of treatment. Full benefit typically develops over 3–6 months of consistent use.

What side effects might I experience?

Injection site reactions — pain, redness, hardness, and bruising — are the most commonly reported effects. They are generally mild and transient. Systemic side effects are uncommon given the targeted mechanism.

Is Ajovy approved for both episodic and chronic migraine?

Yes. Ajovy is FDA-approved for prevention of both episodic migraine (fewer than 15 headache days/month) and chronic migraine (15 or more) — one of the broader approvals among CGRP-targeting migraine preventives.
