How do I know if I have acid reflux or GERD?

Occasional heartburn after a large meal is common and not a disease. GERD (gastroesophageal reflux disease) is diagnosed when reflux symptoms — heartburn, regurgitation — occur two or more times per week, significantly affect quality of life, or cause complications. Persistent symptoms warrant clinical evaluation.

Can acid reflux cause symptoms beyond heartburn?

Yes — GERD can cause chronic cough, hoarseness, throat clearing, asthma worsening, dental erosion, and difficulty swallowing. These 'extraesophageal' manifestations are underrecognized causes of persistent throat and respiratory symptoms, particularly when heartburn is not the dominant complaint.

Do I need an endoscopy?

Most patients with typical, uncomplicated GERD don't require immediate endoscopy — empirical treatment with a PPI is appropriate first. Endoscopy is recommended when symptoms persist despite treatment, when alarm features are present (dysphagia, weight loss, anemia, vomiting), or after age 50 with chronic symptoms to screen for Barrett's esophagus.

Are there lifestyle changes that meaningfully reduce acid reflux?

Elevating the head of the bed, avoiding eating within 2–3 hours of lying down, reducing alcohol and caffeine, losing weight if overweight, and avoiding trigger foods (fatty foods, chocolate, citrus, spicy foods) can meaningfully reduce symptom frequency and severity alongside or before medication.

What side effects are associated with acid reflux treatments?

PPIs are generally very well tolerated; long-term use is associated with modest risk of hypomagnesemia, B12 deficiency, and increased susceptibility to certain GI infections. H2 blockers are safe for short-term use. Antacids are safe but provide only brief relief. Surgical treatment (fundoplication) carries standard procedural risks.
