Achilles tendinopathy refers to pain and degeneration of the Achilles tendon — the large tendon connecting the calf muscles to the heel. It most commonly results from overuse, rapid increases in training load, tight calf muscles, improper footwear, and biomechanical factors. It is more common in runners and middle-aged adults.

What is the difference between insertional and mid-portion Achilles tendinopathy?

Mid-portion tendinopathy involves the tendon body approximately 2–6 cm above the heel — the most common type, treated well with eccentric loading exercises. Insertional tendinopathy involves the tendon at its attachment to the calcaneus — associated with Haglund's deformity and treated differently, as eccentric loading can worsen symptoms at this location.

Will Achilles tendinopathy heal on its own with rest?

Complete rest typically leads to temporary symptom improvement without resolving the underlying tendon pathology — symptoms often return when activity is resumed. Active rehabilitation — particularly eccentric calf strengthening exercises — produces better long-term outcomes than rest alone and is the cornerstone of evidence-based management.

How long does recovery from Achilles tendinopathy take?

With consistent eccentric loading exercise 2 times daily, significant improvement is typically seen at 6–12 weeks. Full resolution may take 3–6 months for mid-portion tendinopathy. Insertional cases and those with more established degenerative change often take longer. Early treatment produces faster and more complete recovery.

What side effects are associated with Achilles tendinopathy treatments?

Eccentric exercises cause expected soreness — moderate discomfort during exercise is acceptable, but sharp or worsening pain requires modification. NSAIDs provide symptomatic relief but carry GI risks. Corticosteroid injection adjacent to the tendon carries a small risk of tendon rupture and is not recommended at the tendon itself.
